Prior to your first visit please complete and print the form above, and take it with you on your first visit. Please write as much as possible so we can best understand what you need to maximize your performance.

How long is each session?

Each session can last up to one hour, so please plan accordingly so we can maximize our time together.

What is the payment policy?

Fees are due at the time of the appointment, and is non-refundable. We accept cash or personal checks only. All returned checks will incur a $25 fee. Cancellations within 24 hours of the scheduled appointment time will be charged at the full amount.
